SBAR-R Communication: The Closed-Loop Read-Back Model

SBAR-R extends the standard 4-step tool by formalizing the 5th critical step: Read-Back / Response (R). Mandated by The Joint Commission for telephone and verbal orders, SBAR-R ensures that critical orders and lab results are mutually verified and free from phonetic misinterpretations.

Why Closed-Loop Read-Back (SBAR-R) Prevents Medical Errors

A clinical recommendation without a verified response leaves communication open-ended. Research demonstrates that up to 37% of verbal orders contain discrepancies if not read back verbatim. SBAR-R introduces four critical closed-loop safeguards:

1. Verbatim Order Read-Back:The receiving nurse reads back the exact medication name, dose, unit of measurement, route, and frequency (e.g., "Confirming: Metoprolol Tartrate, twenty-five milligrams, oral, twice daily").
2. Sound-Alike Drug Prevention:Phonetically similar medications (e.g., Hydralazine vs Hydroxyzine, Celebrex vs Celexa) are spelled out when orders are received verbally over noisy hospital phones.
3. Clear Critical Value Acknowledgment:When communicating critical lab values (e.g., Potassium 6.8 mEq/L), both the lab technician and the nurse document the exact time and receiver verification.
4. Explicit Response & Timeline Commitment:If the provider cannot provide immediate orders, SBAR-R establishes an explicit timeframe: "I will evaluate the patient at bedside in 15 minutes."
